How to Use BASFI Calculator
The BASFI Calculator turns ten simple functional questions into a single score that reflects how much ankylosing spondylitis (AS) limits your daily life. Each question covers a real-life activity: enter a value between 0 and 10 to describe how easy or difficult that activity was during the past week.
- Rate each activity (0–10) — 0 means the activity is completely easy with no difficulty; 10 means impossible or requiring extreme effort. Use the number inputs or type a decimal for a precise rating.
- Answer all 10 questions — The BASFI Calculator requires all ten responses before it can produce a valid score.
- Read the BASFI Score — The calculator displays the average score (sum ÷ 10), an interpretation badge (Mild / Moderate / Severe), and the raw total for your records.
- Copy or reset — Use the Copy button to save the result to your clipboard for sharing with a healthcare provider, or Reset to start a fresh assessment.
The BASFI Calculator is most useful when completed consistently under similar conditions (time of day, recent activity level), so that successive scores can be meaningfully compared.
Formula & Theory — BASFI Calculator
The BASFI Calculator uses the standard formula recommended by the Assessment of SpondyloArthritis international Society (ASAS):
BASFI = (Q1 + Q2 + Q3 + Q4 + Q5 + Q6 + Q7 + Q8 + Q9 + Q10) / 10
| Symbol | Meaning |
|---|---|
| Q1–Q10 | Individual question scores, each rated 0–10 on a numeric rating scale |
| BASFI | Average score, ranging from 0.0 to 10.0 |
Each of the ten questions targets a specific functional domain relevant to AS:
| # | Activity |
|---|---|
| 1 | Putting on socks or tights without help or aids |
| 2 | Bending forward from the waist to pick up a pen from the floor |
| 3 | Reaching up to a high shelf |
| 4 | Rising from an armless chair |
| 5 | Getting up from the floor without help |
| 6 | Standing unsupported for 10 minutes without discomfort |
| 7 | Climbing 12–15 steps, one foot per step |
| 8 | Looking over your shoulder without turning your body |
| 9 | Doing physically demanding activities |
| 10 | Doing a full day of activities at home or at work |
Interpretation
The BASFI Calculator places the score into three broad categories:
- 0–2 (Mild): Minimal functional limitation. Most daily activities can be performed without significant difficulty.
- 2–5 (Moderate): Noticeable limitation. Some activities require adaptation, extra time, or occasional assistance.
- 5–10 (Severe): Significant functional limitation. Many activities are difficult or require substantial help.
Assumptions and Limits
The BASFI is a self-reported index validated for adults with AS. Scores reflect the patient’s own perception over the past week and can be influenced by pain levels, mood, and recent physical activity. The BASFI Calculator is not a diagnostic tool and cannot replace clinical evaluation or imaging.
Use Cases for BASFI Calculator
The BASFI Calculator serves patients, caregivers, and clinicians monitoring AS-related functional status:
- Routine follow-up visits — Track BASFI trends across clinic appointments to document disease progression or improvement after starting a new therapy.
- Treatment response monitoring — Compare scores before and after a biologic medication change to quantify functional benefit.
- Personal health tracking — Patients can use the BASFI Calculator at home to maintain a log and share results with their rheumatologist.
- Research and registries — Clinical researchers include BASFI as a standard outcome measure in AS studies and patient registries.
- Physiotherapy goals — Physiotherapists can set functional goals based on target BASFI scores and use the BASFI Calculator to measure progress after rehabilitation programs.
The higher the BASFI score, the greater the functional limitation. By using the BASFI Calculator consistently, patients and clinicians gain a clear, quantifiable picture of how AS affects daily life.
